With 116 beds and more than 390 physicians on its medical staff, Pali Momi h! as delivered many medical firsts to the community, including West O'ahu's only interventional cardiac catheterization unit for the detection and treatment of heart disease, a fully integrated minimally invasive surgical suite, CT scan and MRI services, emergency services, a Women's Center and the state's first Retina Center. Pali Momi is the only facility in Hawai'i that utilizes a team triage approach in its Emergency Room, where patients are promptly evaluated by an emergency physician. For five consecutive years, the American Heart Association has recognized the hospital with its Gold Award for excellence in the treatment of coronary artery disease and its Gold Plus Award for Heart Failure, making it one of only 7 in the nation to receive this distinction.
The Case Management team puts great emphasis on helping each patient transition their health care treatment with an appropriate set of support tools and information. Our caring, professional staff provides tim! ely medical and discharge management to meet clinical and serv! ice goals, and to provide an effective, appropriate continuum of care. We want to ensure that our patients receive the attention they need beyond the hospital walls, especially when they require care from several different organizations and other combinations of medical, financial or social interventions.
As the Registered Nurse Case Manager, you would ensure the considerate and confidential management of unique and high-risk patient cases by performing case management through direct referrals and reviews of medical records. This would include determining the appropriateness and medical necessity of admissions, continued hospital stay, and the use of ancillary services. You would also perform treatment plan assessments, discharge needs interviews, planning/coordination for high risk cases, and participate in organizational quality improvement activities. We are looking for an individual who is perceptive to patient needs, with outstanding organizational and communi! cation skills and a commitment to delivering the highest quality health care to Hawai'i's people.

Minimum Qualifications: Current Hawai'i Registered Nurse license. Bachelor's degree and/or equivalent combination of education, training, and work experience. Three (3) years recent acute care nursing experience in Pediatrics, OB/GYN, or Adults, or related experience.
Preferred Qualifications: Current Case Management or Utilization Management certification. Case Management experience and knowledge of Interqual.
