This position for a full time licensed pharmacist is located in the Hospital Pharmacy department of Kahuku Medical Center, a 21 bed Critical Access Hospital and Long Term Ca! re Facility in Hawaii, situated on the windward coast, minutes from the North Shore of Oahu.Oversees the hospital pharmacy to ensure that patients' pharmaceutical needs are effectively met. Prepares and dispenses physician medication orders according to policies/procedures and professional standards and practices. Provides consultative services to medical staff, nursing staff, patients, and others. Establishes policies, procedures, and standards and for the provision of services and coordinates with other departments to provide services to patients and staff.

Serves all age groups of patients including neonates, pediatrics, adolescents, adults and geriatrics.Essential Duties and Responsibilities: *Directs, coordinates, supervises and participates in all activities of the medical center pharmacy, including staffing, purchasing, receiving, storing, preparing, dispensing and billing of drugs.*Monitors patient drug therapy regimens for contraindications, allergies, dru! g interactions, appropriateness of drug and dose, therapeutic ! duplications, and adverse reactions.*Provides drug information and education for staff and patients to optimize medication use and provide positive patient outcomes.*Detects, reports, and evaluates suspected adverse drug reactions and medication errors.*Develops department goals and objectives, and establishes and implements policies and procedures for pharmacy operations. Maintains the written drug formulary.*Prepares reports as required.*Participates in medical center and medical staff committee meetings as required.*Recommends department operating budget and ensures that the department operates within the budget.*Supervises, selects, trains, orients and assigns department staff. Develops standards of performance, evaluates performance and makes recommendations for personnel actions.*Demonstrates the skills necessary to provide care appropriate to age-related needs of the patients served.*Complies with all medical center and departmental policies and procedures, safety pr! ogram requirements, corporate compliance program standards of conduct, all applicable statutes, rules and regulations.*Enhances professional growth and development and maintains skills and knowledge through participation in educational programs, current literature, in-services, workshops and professional affiliations.*Performs other related duties as assigned or requested.
