The facility, located in the suburbs of Honolulu, is a 116-bed acute care, community-based hospital that offers the residents of the West Oahu community quality medical care delivered with a unique personal touch. They provide the only cardiac catheterization program in West Oahu and the state’s only Retina Center. The Case Management team is structured to provide timely medical management, utilization management and discharge management interventions to meet clinical targets, to assure the ! provision of a continuum of care to patients, and to meet fina! ncial targets. The Registered Nurse â€" Case Management performs case management through direct referrals and reviews of medical records. Their role is to collaborate with clients by assessing, facilitating, planning and advocating for health needs on an individual basis. Minimum Qualifications: Must meet education requirements for Hawaii state licensing. Current Hawaii RN license. Bachelor's Degree and/or equivalent combination of education, training, and/or experience. Three years acute care nursing experience in Pediatrics, OB/GYN! , or with Adults, or relevant experience. Preferred: Current Case Management or Utilization Management certification. Case Management experience and knowledge of Interqual.
