Hospital Inpatient Setting
Overview of Position
Clinical Pharmacist implements, maintains and provides clinical pharmacist services:
Clinical
Educational
Informational
Distributional functions for newborn, infant, pediatric, adolescent, adult and geriatric patients
You will be asked to perform duties in accordance with state and Federal regulations, Joint Commission standards and policies and procedures.
Job Requirements
Current Hawaii State license as a Pharmacist
Doctor of Pharmacy degree preferred.
Post-graduate clinical residency accredited by the American Society of Health-System Pharmacists preferred.
Hospital Inpatient experience experience preferred.
Preferably with experience in rounding with a medical team, monitoring of patient drug therapy, pharmacokinetic dosing and patient counseling and education.
Experience in unit dose and IV services preferred.
Ability to communicate professional expertise to all levels of the health care team.
