Provides comprehensive Physical Therapy services to children birth to age six using a Transdisciplinary Model. Services are provided to children in their natural environment, inlcluding home, daycare, and community. Reviews clientfiles to obtain background information prior to evaluation to determine appropriate tests and to ensure that adequate information is available. 2. Performs appropriate Multidisciplinary Evaluations and periodic assessments according to age or developmental level of child, areas of concern, program policies and procedures, and an transdisciplinary team approach. 3. Provides direct and consultative therapeutic services to children enrolled in Imua programs according to policies and procedures and individual plans. 4. Counsels family members with respect to the results of the evaluation and makes appropriate recommendations and referrals. Assists families to access community resources. 5. Engages with members of the child’s treatment team to optimize the child’s capabilities and to coordinate physical therapy with other therapeutic activities. 6. Refers appropriate children for mandated medical or other professional services necessary to maximize the developmental progress of children with movement dysfunction. 7. Participates actively in meetings with other team members, including families in order to develop an individualized plan for assigned clients that meet clients' and their families' needs, interest, and abilities. 8. Provides family-centered direct, transdisciplinary and consultative therapeutic services to children enrolled in Imua programs according to policies and procedures and individual plans in order to promote increased sensory motor function through enhancement of musclo-skeletal status, neurobehavioral organization, perceptual and motor development, cardiopulmonary status, and effective environmental adaptation. 9. Provides therapeutic services or treatment to prevent, alleviate, or compensate for movement dysfunction and related functional problems. 10. Provides direct and consultative services with specific regard to obtaining, interpreting, and integrating information appropriate to program planning to prevent, alleviate, or compensate for movement dysfunction and related functional difficulties. 11. Provides consistent and timely feedback regarding the progress and status of clients to other members of the team, including families. 12. Maintains records as required by law and program policies and procedures including: Completes all evaluation reports within the timelines established by written program policies. Documents all communication regarding the client (including phone calls, team meetings, and written correspondence) in daily notes. Completes all documentation necessary to accurately represent billable and non-billable services according to written program policies. Maintain productivity and billing rates as designated by supervisor 13. Provides parent training and consultation in the areas of child development, therapeutic interventions and other topics related to physical therapy and developmental delays as needed. 14. Provides services in Hana andLanaias needed including the completion of the new client intake process in outlying areas while demonstrating an understanding of cultural uniqueness. 15. Presents a positive and professional image to client, families, staff, community agencies, and visitors. 16. Relates to clients and families in a warm, courteous manner. 17. Presents information and recommendations clearly and effectively and communicates in a constructive, supportive manner. 18. Maintains a high level of professional knowledge and competency through participation in meetings, conferences, review of current literature, and other learning activities to promote professional growth in physical therapy and early intervention. 19. Participates in staff meetings and in-services as requested by the Program Director. 20. Conducts in-service presentations at staff meetings on pertinent physical therapy and early intervention at the request of the Program Director. 21. Provides direction in physical therapy procedures and assumes supervision responsibilities of any student providing physical therapy services. 22. Acts as a consultant to educational, medical, and other professional groups. 23. Serves as a consultant to classroom teachers to incorporate physical therapy development activities into daily schedule. 24. Participates, as necessary, in various fundraisers and other agency activities that serve to increase the credibility and visibility of Imua Family Services in the community. 25. Maintains confidentiality of all records and information. 26. Completes other duties and responsibilities as assigned.

Other:
This position may be required to supervise Physical Therapy Assistants, student interns and SupportService Providers as directed. Carries out supervisory responsibilities in accordance with the Agency's policies and procedures and applicable laws. Responsibilities include interviewing, hiring, and training employees as well as planning, assigning, and directing their work, appraising their performance, rewarding and disciplining employees, addressing complaints and resolving problems.

Current and valid driver’s license Current and clean driver’s abstract Current TB clearance First Aid & CPR certification Education and/or Experience Master’s degree in Physical Therapy from an accredited college/university. Pediatric experience with the birth to three population is preferred. Hold a current Hawaii State License in Physical Therapy.
