Be a nurse at an American Air Force Base in Tokyo, Japan!1-year long-term Temp assignment.Salary: Commensurate with ExperienceDisease Manager: Care will cover a range of services that are provided in a civilian disease management facility on an American Air Force Base. • Provides comprehensive Disease Management Nursing Services, applying and promoting disease management, health promotion, and wellness concepts and strategies to assigned patients and those identified for disease Management • Assesses and identifies individual patient care needs and provides the type and amount of care required to meet the individual needs of each patient, making independent nursing diagnoses, providing health care instructions and/or appropriate referral to a provider or other department. • Organizes internal and community resources to provide ongoing self-management support to patients. • Collaborates with providers and patients to define problems, set priorities, establish goals, create treatment plans and implement solutions. • Professional nursing care/treatment will include patients of all ages, pediatric to geriatric and may be conducted via clinic, office, telephone, and electronic contact. Provides advanced nursing services to patients across the population health continuum ranging from normal to those having complex complications requiring sophisticated, highly technical care and treatment. • Sets up and operates a variety of specialized medical equipment such as resuscitators, cardiac monitoring devices, pulmonary function testing, intravenous pumps, ECG units, Pyxis or other medication distribution systems, oxygen analyzers, and nebulizers. • Ensures that work areas are clean and safe according to applicable infection control regulations. • Provides input to educate caregivers and patients on prevention and disease management programs. Formal Education. Degree or diploma from a professional nursing program approved by the legally designated State accrediting agency at the time the program was completed by the applicant is required. Licensure/Registration. The Disease Manager performing services under this agreement must maintain an active unrestricted license from a state or territory of United States or District of Columbia to practice as a registered nurse. English Requirement. The Disease Manager shall read, understand, speak, and write English well enough to effectively communicate with patients and other Health Care Workers (HCW). Experience. It is preferred that the Disease Manager have at least one year of pharmacy experience within the past two years practicing as a Disease Manager. However, this is not a requirement for award and will not be evaluated during the solicitation.
